Badge Replacement Request

  • Request for Employee Badge Replacement (For Existing Employees Only)

    To request a replacement badge, please complete the form on the below listed link:

    • Please allow at least 24-48 hours for your request to be processed.

    If your badge no longer works (swipes) please inter-office mail your badge to the Department of Human Resources to be re-magnetized. You may also bring your badge to the Administration/Education building Monday-Thursday from 3:00 PM - 4:00 PM for a replacement.  

    NOTE: Your badge will be sent to your campus/department secretary within 24-48 hours of your request, unless otherwise noted on your form.
